Privacy Complaints

UNM takes its commitment to privacy seriously, and strives to address privacy complaints in a timely manner. Privacy complaints can be submitted as follows:

Reporting a HIPAA Privacy Complaint

To report a HIPAA privacy incident to the UNM Health Sciences Center Privacy Office

• Complete the “Report an Incident” form at: Report an Incident
• Complete the incident report form at: Incident Notification Report
  • And submit either via email to HSC-Privacy@salud.unm.edu
  • Or via fax to 505-272-2461
• Call 505-272-1493

Reporting any non-HIPAA Privacy Complaint

• Make a report at unm.ethicspoint.com
